CORI Form (Criminal Offender Record Information)

A blank CORI form is attached for your convenience.

Simply print the form, complete the form and bring it to the Recreation Office along with a valid government issued picture ID.

It is a State requirement that all individual indentities are verified in person.

The Recreation Office is located at Town Hall, 62 Arlington Street, 2nd Floor.

Town Hall Hours of Operation:  M - W - Th 8:30am - 4:30pm, Tuesday 8:30am - 7:00pm, Friday 8:30am - 1:00pm
